Shareholder Information
UIC Foundation is a non-profit organization committed to providing financial aid to eligible students pursuing either a degree or
certification. Our goal is to provide our Shareholders and their descendants support to receive the education that they need
to pursue careers in the Community and abroad. To find out more information regarding our programs and services, please e-mail

For Renewals:
-Application filled out
-Fall grades
-Financial aid need sheet completed by the school the person is attending
For those signing up for Spring semester I need the entire application filled out plus:
-Acceptance letter from the school that they will be attending
-Complete transcripts from either your high school or the most recent school you have attended
-Three (3) letter of recommendations
-A Personal statement detailing your future career plans after completion of college or training
-Financial need sheets completed by your/their school of choice
